MEMBER AGREEMENT

THIS AGREEMENT is between

and Utransform LLC, a limited liability company based in the State of Arizona. You, the above-named person acknowledges that all the information you are providing on this form is true and accurate. You the above-named registering Member of Utransform.org are also agreeing to follow a set plan aimed at helping you to become metabolically healthy. This set plan will be emailed to the email address you have provided above. Make sure you keep a copy for your records.

Utransform LLC and Utransform.org agree to provide you with the education and action steps for you to achieve metabolic health according to your Healthcare Provider's recommendations. We at Utransform LLC and Utransform.org agree to provide you with a plan within 72 hours of receiving this signed agreement to put you on the road to metabolic health. If you do not receive a plan emailed to the address you have put in this Agreement by 72 hours text or call Dale at 208-241-7438.


By signing this Agreement below you are acknowledging you understand any and all legal implications as well as what this Agreement is all about. You agree that Utransform LLC nor Utransform.org will be legally responsible or medically responsible for any of your actions. You are agreeing that YOU and YOU alone are responsible for ALL of your actions. By signing this Agreement you have either consulted with a legal representative or you fully understand all the implications within this Agreement. IF YOU ARE IN AGREEMENT PLEASE SIGN BELOW:
